Systemic failures in care of woman who experienced three antepartum haemorrhages - 21HDC00221

In a decision released today, health and disability commissioner Morag McDowell has found Auckland DHB breached the Code of Health and Disability Services Consumers’ Rights (the Code) for failures in the care provided to a pregnant woman during her stay at Auckland Hospital, including poor management and planning when she was discharged.

A registered midwife has also received adverse comment for a lack of detailed progress/notes during the woman's pregnancy.
